Objective

The intention of the work in the Data Integration Center (DIC) is to enable medical information to be collected, stored and exchanged in a form in which it can be optimally used for healthcare and research. The services of the data integration center contribute directly to the establishment of a research-compatible electronic patient record at the Jena University Hospital.

Establishment of the Data Integration Center

As part of the Medical Informatics Initiative of the Federal Ministry of Education and Research, university hospitals in Germany are establishing Data Integration Centers (DIC). Their aim is to make research data and healthcare data available on a larger scale and are linked to each other (see project description "SMITH").

The establishment of the Data Integration Center as an organizational and technical connection point between clinical care and scientific research is performed at the Information Technology Department in close cooperation with the Institute for Medical Statistics, Informatics and Data Sciences (Prof. Dr. André Scherag as scientific director and Prof. Dr. Cord Spreckelsen).

Tasks and Services of the Data Integration Center

  • establish access to healthcare or research data collected or documented decentrally at the Jena University Hospital
  • transformation of these data from heterogeneous formats into interoperability standards including standardized semantic annotation (IHE profiles, HL7 CDA, HL7 FHIR, LOINC, SNOMED CT etc.)
  • development of a digital patient record and synchronizable metadata directories for structured documentation (in accordance with the project-oriented IT platform strategy to establish interoperability at the Jena University Hospital)
  • development of a platform similar to the patient record for the permission-based usage of interoperably structured patient data in the context of biomedical research ("Health Data Storage")
  • acquisition of third-party funds through the initiation of and participation in research and data use projects,
  • support of the active participation and informed consent of the patient to the use of her or his data
  • quality assurance and curation of accessible healthcare data
  • operation of IT procedures for  data exchange and distributed data use with external partners, e.g. data registers
  • support of data analysis and processing procedures from feasibility studies to patient-specific clinical decision support
  • possible retransfer of analysis results into the clinical primary documentation and use in general healthcare
  • support and further development of and consulting on procedural, syntactic and semantic interoperability standards 
  • support and advice on the integration of IT processes and IT infrastructure in UKJ project proposals funded by third parties
  • support and training for users of the services of the Data Integration Center
  • cooperation with other Data Integration Centers and Standards Developing Organizations to further develop data use, software development and interoperability standards in healthcare

Use and Access Committee

A Use and Access Committee is responsible for managing and controlling the use of healthcare and research data in the context of projects. The decisions of this committee on the implementation of project applications (application review, data use, data access) are made in close cooperation with the Ethics Committee of the Friedrich-Schiller University Jena at the Medical Faculty, and with the Legal Department of the UKJ, with the official data protection officer of the UKJ, and with the structural units / projects / consortia contributing data.

SMITH – Smart Information Technology for Healthcare

In a research consortium, clinical, epidemiological, and medical systems researchers, medical informaticians, as well as Chief Information Officers and IT staff members from the university hospitals of Aachen, Bonn, Düsseldorf, Essen, Halle, Hamburg-Eppendorf, Jena, Leipzig, und Rostock work closely together with non-university research institutions and industry partners. Within the Data Integration Centers, these groups strengthen a sustainable culture of interdisciplinary cooperation and quality-assured structured medical documentation and make specialist knowledge and research findings available throughout Germany. The consortium is named SMITH – Smart Medical Information Technology for Healthcare. 
At the Jena University Hospital, the IT infrastructure will be developed to make healthcare data accessible to clinical research and usable e.g. for decision support systems. The Data Integration Center will support structured medical and study documentation in internationally standardized formats (HL7 CDA, HL7 FHIR, etc.) for curation and analysis, and enables the exchange of these by means of internationally standardized communication and security procedures (IHE). At the same time, in the Data Integration Center conditions are created which enable new findings from inter-institutional research to reach patients directly.

Cooperation with Standards Developing Organizations

The Data Integration Center, within the Jena University Hospital and as a part of the SMITH consortium, works closely together with the German organizations responsible for the development of technical standards for digitalization in healthcare (SDOs, Standards Developing Organizations).

These include in particular HL7 Deutschland e.V. and IHE Deutschland e.V.

Within the framework of the IT platform strategy of the Jena University Hospital Jena and for all data usage and data exchange projects at the Data Integration Center, internationally agreed interoperability standards are used. On the basis of modern technical formats and protocols as well as through globally unique encoding of medical contents, these standards enable automated processing of medical data independent of location and manufacturer, and are thus the key to the progress of digitalization in the health care system.

Data Use Project "WE-STORM"

Project Name: Weather-based Stroke Event and Outcome Risk Modeling

Abstract:
Heat waves are known to lead to an increase in hospital admissions. The present project aims to investigate how great the influence of heat is on a person's risk of stroke. For this purpose, the first step is to determine a baseline risk that patients have in general and depending on their health status. The second step is to determine how much the risk of stroke increases as a result of a heat wave in a region. For this purpose, regional data from the German Meteorological Service are used and analyzed together with data on admissions with a diagnosis of stroke from many participating university hospitals.

Project Name: NT-proBNP as a Marker in Atrial Fibrillation

Abstract:
It is sometimes difficult and tedious to reliably diagnose atrial fibrillation, a mostly chronic cardiac arrhythmia, in patients. Ususally, a long-term ECG (electrocardiogram), i.e. a measurement of the heart currents, is used for this purpose. In the NT-proBNP project, the aim is to determine whether the measurement of the biomarker NT-proBNP can be used in addition to the ECG as an indication for a reliable diagnosis. (A biomarker is a biological characteristic that can be measured in blood or tissue samples). For this purpose, the correlation between atrial fibrillation and the occurrence of the biomarker NT-proBNP will be analyzed at all participating university hospitals.

Abstract:
For an overview of relevant parameters of COVID-19 cases at the university hospitals, a web dashboard was developed which can be filled with aggregated data from multiple institutions. With the hospital and ITS durations, the dashboard already contains current data that is otherwise hardly available. Cumulated data for all participating university hospitals is presented publicly.

Abstract:
In the POLAR project, medical informatics specialists, biometricians, epidemiologists, pharmacists, clinical pharmacologists and healthcare researchers from 21 institutions, including 13 university hospitals, are working together to collect data on prescribed drugs (e.g. medication plans) as well as on prescriptions and drug dispensations from pharmacies, to classify polymedications with regard to Potentially Inadequate Medication (PIM) and a selected range of drugs as high-risk prescriptions, to digitally represent scores to identify high-risk patients for drug-related problems and to identify the occurrence of adverse drug reactions and their consequences at an early stage or to avoid them altogether.

Data Use Project "HELP"

Project Name: A Hospital-wide EMR-based Computerized Decision Support System to Improve Outcomes of Patients with Bloodstream Infections

Abstract:
In the use case HELP, we are developing a hospital-wide computerized decision support system to improve the outcomes of patients with bloodstream infections. This involves the targeted use of antibiotics in accordance with clinical guidelines to combat bacterial infections – especially against the background of the insufficient number of trained infectiologists in Germany. The use case will be implemented and evaluated on normal and intensive care units at the SMITH locations Jena, Leipzig and Aachen as well as Halle and Essen.

Abstract:
In the Use Case ASIC, an algorithm-based monitoring of the condition of critically ill patients will be instituted in intensive care units by means of continuous evaluations from the patient data management system. This will create the prerequisite for an early alarm in case of acute lung failure (ARDS), which should enable rapid diagnostic and therapeutic intervention. These data are analyzed by high-performance computing and processed for clinical decision support.

Abstract:
In the STAKI2B2 project, valid phenotype data and other relevant comparative information will be extracted automatically from clinical documents using automatic speech processing methods to improve the application of new markers from biomaterials in clinical processes. For this purpose, a text analytics pipeline will be established, which automatically determines relevant medical entities and relationships between these entities from clinical documents using semi-monitored machine learning methods.

Abstract:
The aim of the project is to investigate the transmission of multi-resistant gastrointestinal pathogens in health care systems of selected countries in Europe and Israel. The role of patient flows between hospitals and during inpatient stays as well as the characteristics of different gastrointestinal pathogens will be considered. Based on this, a generic network model will be developed to investigate the effects of different prevention strategies.

Abstract:
The aim of the present study is a retrospective descriptive evaluation of the topics multimorbidity and rare diseases based on available billing data of the participating university hospital sites. The data will be used to determine comorbidity scores and to compare them with previously published analyses. In addition, for aggregated rare diseases, statements on the distribution and distance of care to the participating university hospitals will be described with the help of a geovisualisation, while respecting data protection.
