Follow-up care
As part of discharge management in accordance with Section 39 (1a) SGB V, our social services staff will work with you to take care of your follow-up care - usually during your inpatient stay with us.
This includes
- Organization of follow-up treatment / rehabilitation (outpatient and inpatient)
- Procurement of aids (wheelchairs, walking aids, orthoses, prostheses or similar)
- Organization of home care
- Initiation of physiotherapy and occupational therapy
- Advice, also together with relatives or caregivers
If necessary, the health or care insurance fund will support the follow-up treatment.
Further treatment
After your discharge, please see your family doctor or the doctor who referred you to the UKJ as soon as possible. He or she will usually receive a written letter from us a few days after your discharge with all the important information for your further treatment. Please note: The hospital is not liable for any further consequences for patients who insist on being discharged against medical advice or leave the hospital without authorization.
Would you like to request your treatment records? Here you will find the relevant form (German), which you should complete and return to the UKJ Legal Department.
Nothing forgotten?
Before you leave your ward room, make sure that you have packed all your personal belongings. Do you have any valuables left behind or a bill to pay? Remember to return borrowed items, books and magazines to us in good time.
If you have forgotten something in your ward room or during an examination, please ask on the respective ward or in the functional area. You are also welcome to contact our central lost and found office on 03641 9-3 32 12.

Please keep the receipt/payment slip in a safe place, as in the event of a further hospital stay in the same calendar year, you will only be required to make a co-payment if the maximum amount was not reached during the previous hospital stay.
